Project Description

Lana\u2018i Community Health Center (LCHC) is a Federally Qualified Health Center (FQHC) whose mission is to take care of the Lana\u2018i community physically, and mentally. LCHC carries out this mission by working collaboratively with community partners to provide direct and comprehensive health and wellness services to island residents of all ages, ethnicities and genders. Through the delivery of a wide range of health services, LCHC ensures access to residents experiencing significant health disparities. \n\nLCHC\u2019s target population consists of everyone on Lana\u2018i, with a special focus on those who live at or below 200% of the federal poverty level. LCHC provides services in a culturally sensitive manner \u2013 providing written and oral translation as needed. No one is turned away due to an inability to pay for services, we accept all insurances and provide assistance to our patients with understanding and navigating any insurance coverage questions they may have. \n\nSince LCHC was established in 2008, our patient base has grown to serve 90% of the island's population or 3,013 residents. LCHC provides quality clinic, home, work and school based services offering both ongoing and episodic care. These services are provided to a rural community that is classified as a medically underserved population where the per capita family income is $29,347 and 11.2% of our patients are uninsured. \n\nLCHC is one of the major island employers, currently employing 62 staff. We hire Low-Income Peoples (LIP) and provide them with workforce and development training, of our current staff 58.1% were defined as LIP at the time of hire. LCHC also contracts over a dozen providers, fitness instructors and thirty vendors. To ensure that everyone in Lanai is receives the services that they need this facility must be expanded.
