University Community Health Services, Inc. d/b/a Connectus Health
Connectus Health provides healthcare services and social support services for all in need, regardless of ability to pay. Patients and community members served are often low income, uninsured, or underinsured. Additionally, many patients and community members served are part of disadvantaged populations such as racial and/or ethnic minorities and the LGBTQIA community. Many Connectus patients struggle with access to healthcare resources that are affordable, culturally sensitive, and gender inclusive. As a result, many patients avoid healthcare services and are often in advance stages of illness when finally seeking care. By providing low-cost, high-quality, culturally sensitive, and gender inclusive healthcare and social support services for some of the most disadvantaged in the community, Connectus Health is supporting a healthier and safer community and contributing to the containment of uncompensated medical costs and unnecessary emergency room and hospital services.

Procurement pursuit intelligence
This project has been enacted. FFID maps funded projects to likely downstream procurement channels — including recipient portals, state and local systems, agency programs, and Grants.gov. SAM.gov is monitored, but many CPF/CDS-funded opportunities appear first through recipient or state/local procurement systems.
